SERVICE

One or more organization-operated programs or activities that have a common general objective and deploy the organization's material and human resources in a planned and systematic manner. An organization that publicly promotes or identifies itself in writing as offering a service, is licensed to deliver a service, assigns personnel and/or space to a service, or allocates financial resources to a service is considered to offer that service.

  FAMILY

Two or more people who consider themselves family and who assume obligations, functions, and responsibilities generally essential to healthy family life. Child care and child socialization, income support, long-term care, and other caregiving are among the functions of family life. The definition of "family" will rest with an individual's indication of who plays a family member role, including current or former foster family, adoptive family, extended family members, fictive kin, or significant others. Organizations that believe family is the central constellation in a child's life, and that family attachments are of primary importance for human development, will strive to work with professional staff to develop a common understanding of "family."

  SERVICE PLAN

A written plan of action based on the assessment of consumer needs and strengths that identifies problems, sets goals, and describes a strategy for achieving those goals and engaging in joint problem solving with the consumer. Also known as a "treatment plan".

  PROGRAM

A system of services offered by an organization. For example, an organization providing a mental health service may offer several mental health programs to different populations, e.g., a mental health program for adolescent teens. The word "program" can be used interchangeably with the word "service" or to describe specific programs.

  CASE RECORD

A written compilation that describes the client and the services delivered. Records can be in hard copy and/or electronic format. The case record can be used as a source of information for quality improvement or other evaluation activities, for research purposes, or to demonstrate accountability to funding bodies.

  PRACTICE

Established actions or ways of proceeding in the regular performance of organizational duties. Policies and procedures often guide practice.

PA-DDS 9: Residential Services

 
The agency protects the rights, and provides for the comfort of individuals using residential services.
NA The agency does not provide residential services.

PA-DDS 9.01

 

The agency provides a living environment that:

  1. is home-like;
  2. provides the individual the opportunity to design or personalize his or her space;
  3. respects the person's right to privacy; and
  4. provides the individual with enough space to spend time alone.

PA-DDS 9.02

 
Individuals receiving residential services have the opportunity to visit and receive visits from family and friends, and the agency assists with travel arrangements, as necessary.

PA-DDS 9.03

 
Individuals receiving residential services have the right to make private telephone calls at any time.

Interpretation: Any restrictions on the right to make private phone calls are:

  1. based on contraindications in the service plan;
  2. approved in advance by the program director or an appropriate designee;
  3. documented in the case record; and
  4. reauthorized weekly by the immediate supervisor of the direct support provider.

PA-DDS 9.04

 

Individual are free to express and practice their religious or spiritual beliefs, and the agency provides:

  1. appropriate opportunities for religious or spiritual practice; and
  2. support, including transportation and schedule adjustments, as necessary.
