PA-YIL 6: Service Coordination for Youth with Special Needs

Youth with
special needs receive additional services that are integrated into a coordinated, goal-oriented
service plan designed to promote safe and stable living, and build independence.
Interpretation: “Youth with special needs” can include those with developmental disabilities and communication barriers, separated and unaccompanied minors, and
immigrant and
refugee youth.
NA The agency does not serve youth with special needs.
Service coordination for youth with special needs includes:
- a plan for independence developed with the young person, if appropriate;
- support for the person’s steps toward maximum independence in the least restrictive environment; and
- maintaining or strengthening family connections, if appropriate.
Together with the young person, the agency develops an inventory of competencies in personal and life skills that is:
- given to the person served;
- the basis for ongoing review and documentation of progress; and
- noted in the case record as part of the service plan.
Interpretation: The agency considers the utility of available, standardized life skills
assessment tools designed to measure and monitor areas of strength and needed improvement for this population, and whenever possible uses a tested instrument. Designing an instrument, and not using a structured measure, are not recommended
practices.
The agency develops a plan for independence based on a skills inventory that enumerates measurable goals, and serves as a
performance agreement or
contract between the youth and the agency.
