COMMUNITY

A specific group of people living in the same locality and who may share a common culture, values, and norms. Communities can also be defined by race, religion, ethnicity, age, occupation, political status, tribal affiliation, interest in particular problems or outcomes, or other common bonds. The term "community" encompasses worksites, schools, tribes, residential neighborhoods, business districts, recreational areas, and health and human service sites.

  SERVICE

One or more organization-operated programs or activities that have a common general objective and deploy the organization's material and human resources in a planned and systematic manner. An organization that publicly promotes or identifies itself in writing as offering a service, is licensed to deliver a service, assigns personnel and/or space to a service, or allocates financial resources to a service is considered to offer that service.

  RESEARCH

For purposes of COA accreditation, all forms of internal or external research involving persons served except internal program evaluation and outcomes research, or educational projects performed by students and interns that are part of their professional training.

  PROGRAM

A system of services offered by an organization. For example, an organization providing a mental health service may offer several mental health programs to different populations, e.g., a mental health program for adolescent teens. The word "program" can be used interchangeably with the word "service" or to describe specific programs.

  ADVOCACY

An act performed with or on behalf of others through direct intervention, empowerment, or representation. Case advocacy refers to actions taken in relation to a particular individual consumer. Cause, social, or systems advocacy refers to actions taken in relation to a common issue affecting a group of persons.

  CASE

A general term used to designate clients (including individuals, families, and groups) served by an organization for purposes of monitoring the provision of services. A foster care case is generally based on the placement of an individual child, although casework for the child may include services to the child's family. A child protective services case is based on an entire family household if a family assessment model is used; otherwise a case is defined as a child.

The misuse of a chemical substance in a manner that is detrimental to an individual's physical or mental health.

Ongoing review and oversight of a client's use of prescribed or over-the-counter medications by a physician or other prescribing clinician.

  MONITORING

An evaluation involving a periodic review of consumer services, organizational activities, or conduct. Specifically, monitoring is an activity of case coordination, whereas more broadly, monitoring is an evaluation technique used in overall quality assurance.

  PREVENTION

Actions taken to minimize and/or eliminate social, psychological, or other conditions. Prevention can occur at the individual, group, community, and societal levels and enhances opportunities to achieve positive fulfillment.

  FAMILY

Two or more people who consider themselves family and who assume obligations, functions, and responsibilities generally essential to healthy family life. Child care and child socialization, income support, long-term care, and other caregiving are among the functions of family life. The definition of "family" will rest with an individual's indication of who plays a family member role, including current or former foster family, adoptive family, extended family members, fictive kin, or significant others. Organizations that believe family is the central constellation in a child's life, and that family attachments are of primary importance for human development, will strive to work with professional staff to develop a common understanding of "family."

The process of specifying objectives, evaluating the means for their achievement, and exercising deliberate decision making about appropriate courses of action.

The immediate response to the acute needs of a person in crisis including referral to appropriate community resources, advocacy, support, or direct assistance.

SCL 6: Service Components

 
The organization provides community-based, safe, and accessible housing, and services that encourage the development of personal support systems and independence.
Research Note: Research suggests that “Housing First”, Safe Haven programs, and other low demand housing models where use of services are not required, are effective at achieving positive housing outcomes for the chronically homeless. Often these programs accept people who may not be "housing ready," or for whom standard housing models have proven ineffective.

SCL 6.01

 
Supported Community Living Services are provided in community settings that are readily accessible to public transportation, shopping, and community services.

SCL 6.02

 

Housing services include:

  1. information about community housing options;
  2. assistance obtaining a safe, stable living environment;
  3. education on tenant rights and responsibilities;
  4. a process to alert service staff when an individuals' rent is overdue and to help prevent rent arrears from mounting; and
  5. advocacy for safe, affordable, appropriate housing.

SCL 6.03

 
The organization provides case management and services that are individually tailored to the needs of residents.

SCL 6.04

 

Skills training to promote independence, development of personal support systems, and/or housing readiness, includes:

  1. activities of daily living;
  2. household management;
  3. budgeting and money management;
  4. credit and debt counseling;
  5. personal safety;
  6. the use of community resources;
  7. information about public assistance; and
  8. interpersonal communication.

SCL 6.05

 

Health services include:

  1. routine medical and dental care;
  2. substance abuse and mental health services;
  3. medication management and/or monitoring;
  4. developmentally appropriate information, including pregnancy prevention, family planning, and information about prevention of HIV/AIDS and STDs; and
  5. harm reduction.

SCL 6.06

 

Support services include, as needed:

  1. educational services;
  2. crisis intervention;
  3. transportation;
  4. legal assistance;
  5. case advocacy;
  6. parent education and family support;
  7. child care; and
  8. social, cultural, recreational, and religious activities.

SCL 6.07

 

Employment and vocational support includes:

  1. developing work habits, skills, and self-awareness essential to employability;
  2. writing resumes, completing job applications, and preparing for interviews; and
  3. finding and accessing local employment resources and placement options, including on-the-job training.
