SERVICE

One or more organization-operated programs or activities that have a common general objective and deploy the organization's material and human resources in a planned and systematic manner. An organization that publicly promotes or identifies itself in writing as offering a service, is licensed to deliver a service, assigns personnel and/or space to a service, or allocates financial resources to a service is considered to offer that service.

  ASSESSMENT

An evaluation, which utilizes professional expertise and skills in the collection and analysis of data to understand and describe the nature of service needs of an individual, family, or group. Assessment, as in needs assessment, is also used to determine priorities of program planning and service development for the organization as a whole. See also DIAGNOSIS.

  SERVICE GOALS

Broad, issue-oriented statements that reflect the realistic achievements to be accomplished in the short or long term. Goals are achieved through the accomplishment of specific quantifiable objectives.

Systematically developed, objective, and quantifiable statements used to assess the appropriateness of specific decisions, services, and outcomes.

AG 4: Assessment

 

The individual participates in a comprehensive, individualized, and culturally-responsive assessment to identify:

  1. his or her values and beliefs;
  2. service needs and desired outcomes;
  3. resources to support the achievement of service goals; and
  4. strengths and informal supports.

Interpretation: Culturally responsive assessments can include attention to geographic location, language of choice, and the person’s religious, racial, ethnic and cultural background. Other important factors that contribute to a responsive assessment include attention to age, sexual orientation, developmental level, and cognitive abilities.
Interpretation: Service needs can include housing, financial support, vocational services, habilitation services, physical and occupational therapy, medical and dental care, aide service, education, social supports, and recreational activities.
Note: Refer to the Assessment Matrix for additional Comprehensive Basic Assessment criteria. The elements of the matrix can be tailored according to the needs of specific individuals or service design.
